- State employees or officials whose annual salaries surpass the threshold or who are designated policy-makers after the applicable filing due date must file within 30 days of the change in their salary or policy-maker designation. New State employees or officials who (1) have annual salaries above the threshold or are designated policy-makers, and (2) commence employment after the applicable filing due date must file within 30 days of hire.

2024 Exemption Application (Individual)
A filer who is not designated as a policymaker and whose annual salary exceeds the SG-24 filing rate may request an exemption from filing a FDS. Such exemptions may be requested through the FDS Online Filing System or by completing and submitting this writeable PDF form, which must be received or postmarked on or before the filing due date.

2024 FDS Agency Exemption Application
State agencies and labor unions may request an exemption from filing a FDS on behalf of individuals in the same title or job classification who are not designated as policy makers and whose annual salary exceeds the SG-24 filing rate. Such exemptions may be requested through the FDS Online Filing System or by completing and submitting this writeable PDF form, which must be received or postmarked on or before the filing due date.

2024 FDS Extension Application
A filer may request a 45-day extension of time to file the FDS based on justifiable cause or undue hardship. 2024 Application for Deletion or Exemption
A filer may apply to the NYS Commission on Ethics and Lobbying in Government for deletion of specific information from public inspection or exemption from reporting certain information regarding a spouse or child.

2024 FDS Spousal Affidavit
State officers and employees required to file a FDS may also be required to provide information regarding their spouses. Filers with extenuating circumstances that prevent them from obtaining information regarding a spouse may utilize this form and submit to the Commission on Ethics and Lobbying in Government.

Guidelines to the Random Review Program
Executive Law § 94(9)(n) authorizes the Commission to conduct a program of random reviews of Financial Disclosure Statements submitted to the Commission pursuant to Public Officers Law § 73-a, and requires that this program be carried out in a manner detailed in these guidelines.
